Defeated - Add a 0.01 ETH pool with an enforced DAO fee (phase 1 of 3)

This proposal deploys and registers a new 0.01 ETH pool, FeeEnforcedTornado_eth, whose protocol fee cannot be bypassed. Today the 0.3% DAO fee is only collected when a withdrawal goes through a registered relayer; withdrawing directly from the pool, or through an unregistered relayer, pays the DAO nothing. The new pool charges the fee inside the withdrawal itself: withdrawals through a registered relayer keep working exactly as today (0.3% burned from the relayer's TORN stake, no extra charge), and every other withdrawal pays 0.3% + 0.3% premium of the note in ETH, for the TORN lockers. Governance will then receive TORN and ETH as claimeable. Governance can change both fees values later, up to hard caps of 1% and 4%. The pool is registered as ENABLED in the InstanceRegistry with the same 0.3% protocol fee as the live pools, and the FeeManager fee is refreshed in the same transaction. Nothing else is touched: no live pool, the Router, the RelayerRegistry, the staking contract or Governance. This is the first of three proposals, meant to be tried on mainnet with small amounts: 1. This proposal: the 0.01 ETH pool. Until the staking contract is upgraded, the pool holds the ETH fees it collects; nobody can take them out, and anyone can forward them to the staking contract once it accepts ETH. 2. Staking upgrade: TornadoStakingRewards takes the ETH fees and shares them among TORN lockers. The code is ready in the repository, for review. 3. Rollout: the other pools (0.03, 0.3 and 3 ETH) and the interface update.
